If you did not receive a Class Notice and Claim Form in the mail, a blank Claim Form is available here. There are three ways you may submit your Claim Form:

Option 1: By Mail

Submit your completed Claim Form by mail postmarked by October 10, 2019 to:

Option 2: Secure File Upload

Submit a PDF of your completed Claim Form using Analytics’ secure file upload system October 10, 2019:

Create an account and prepare and submit your Claim Form online by October 10, 2019.
